1. Przedmiotem zamówienia jest realizacja zdania pn.: Pełnienie kompleksowego nadzoru do zadania inwestycyjnego pn. „Przebudowa i rozbudowa Kliniki Psychiatrii Centralnego Szpitala Klinicznego Uniwersytetu Medycznego w Łodzi bud. B1 ul. Czechosłowacka 8/10" w trybie zaprojektuj i wybuduj dla Centralnego Szpitala Klinicznego Uniwersytetu Medycznego w Łodzi przy ul. Pomorskiej 251. (Inwestycja). 2. Inwestycja realizowana w ramach Umowy o dofinansowanie DOI/FM/SIS/PSYCH/2026/305/51 na udzielenie dotacji celowych na dofinansowanie realizacji programu inwestycyjnego pn. „Przebudowa, rozbudowa i doposażenie Klinik Psychiatrycznych Centralnego Szpitala Klinicznego Uniwersytetu Medycznego w Łodzi”. Program Inwestycyjny został ustanowiony Uchwałą nr 8 Rady Ministrów z dn. 8.01.2026r. Wykonawca zobowiązany jest do uwzględnienia wszelkich rygorów i procedur sprawozdawczych wynikających z faktu finansowania zadania ze środków zewnętrznych. 3. Szczegółowy opis przedmiotu zamówienia zawierają odrębne załączniki - plik OPIS PRZEDMIOTU ZAMÓWIENIA (OPZ) w części B SWZ. Miejscem wykonania Zamówienia jest: Samodzielny Publiczny Zakład Opieki Zdrowotnej Centralny Szpital Kliniczny Uniwersytetu Medycznego w Łodzi z siedzibą w Łodzi w lokalizacji w Łodzi bud. B1 ul. Czechosłowacka 8/10. 4. Oznaczenie wg Wspólnego Słownika Zamówień CPV: 71247000-1 Nadzór nad robotami budowlanymi; 71540000-5 - Usługi zarządzania budową -- pozostałe CPV w opisie przedmiotu zamówienia 5. W przypadku niniejszego postępowania Zamawiający odstąpił od podziału na części ponieważ uznał, że brak jest możliwości podzielenia zamówienia na części. Zamawiający nie podzielił zamówienia na części ze względu na charakter zamówienia (dalej w swz.). 6. Zamówienie będzie realizowane w okresie maks. do 31.12.2027 r. 7. Zakres i warunki zmian zawarto we wzorze umowy, stanowiącym załącznik do SWZ. 8. Pozostałe zapisy zg. z SWZ.
