1.Przedmiotem zamówienia jest realizacja zadania pn.: „Dostawa zestawu podstawowych urządzeń do przygotowania radiofarmaceutyków w ramach umowy DOI/SK/85112/6220/305/2026/680 na udzielenie w 2026 r. dotacji celowej na zakupy inwestycyjne w SP ZOZ CSK UM w Łodzi” 2. W zakres przedmiotu zamówienia wchodzą, w szczególności: urządzenie do wizualnej oceny radiofarmaceutyków, urządzenie do badania integralności rękawic, stół medyczny ze stali nierdzewnej, półautomatyczny deyspenser radiofarmaceutyków, pojemnik na odpady beta promieniotwórcze, miernik aktywności promieniotwórczej, komory wstępne, komora typu izolator, komora typu biohazard w ilościach określonych w Załączniku Nr 2 do SWZ. Ze względu na obszerny opis - szczegółowy opis przedmiotu zamówienia, szczegóły realizacji przedmiotu zamówienia itp., zawarte są w załączniku nr 2A do SWZ - Opis Przedmiotu Zamówienia - Parametry Techniczne (dalej zwany „OPZ”). 3.Wykonawca, zobowiązany jest załączyć do OFERTY w szczególności Załącznik Nr 2 oraz nr 2A do SWZ - OPZ - Parametry techniczne uzupełniony zgodnie z wymaganiami Zamawiającego. 4.Zamówienie obejmuje 1 pakiet (część). Zamawiający nie dopuszcza składania ofert częściowych. Wykonawca może złożyć wyłącznie 1 ofertę na przedmiot zamówienia. W przypadku niniejszego postępowania Zamawiający odstąpił od podziału na części ponieważ uznał, że brak jest możliwości podzielenia zamówienia na części - dokładne uzasadnienie w SWZ. 5.Realizacja zadania w ramach umowy DOI/SK/85112/6220/305/2026/680 na udzielenie w 2026 r. dotacji celowej na zakupy inwestycyjne w SP ZOZ CSK UM w Łodzi 6.Dostarczony przedmiot zamówienia musi posiadać instrukcje obsługi w języku polskim (i ile są wymagane), legitymować się stosownymi dokumentami, o których mowa w SWZ, musi być objęty co najmniej okresem gwarancji - określonym dla danego urządzenia, sprzętu, określonym w załączniku Nr 2 oraz nr 2A do SWZ. W przypadku powzięcia wątpliwości co do spełnienia parametrów technicznych przez oferowany sprzęt, urządzenia, Zamawiający jest uprawniony do żądania dokumentów potwierdzających oferowane parametry techniczne sprzętu, urządzeń np. karty katalogowe, certyfikaty itp. 7.KOD CPV: 33151000-3 8.Zamawiający informuje, zgodnie z art. 441 ust. 1, iż nie korzysta z prawa opcji. 1.Przedmiotem zamówienia jest realizacja zadania pn.: „Dostawa zestawu podstawowych urządzeń do przygotowania radiofarmaceutyków w ramach umowy DOI/SK/85112/6220/305/2026/680 na udzielenie w 2026 r. dotacji celowej na zakupy inwestycyjne w SP ZOZ CSK UM w Łodzi” 2.
