Description

Przedmiotem zamówienia jest zakup aparatu USG i wózków transportowych na potrzeby SPZOZ WSS nr 3 w Rybniku. Umowy w zakresie rozstrzygniętych pakietów zostaną zawarte bezpośrednio po zakończeniu postępowania. Zadanie uzyskało dofinansowanie ze środków budżetu Województwa Śląskiego (dotacja celowa na zadanie inwestycyjne pn.: „Zakup stołu i lamp operacyjnych, aparatu USG i wózków transportowych na potrzeby SPZOZ WSS nr 3 w Rybniku”). Szczegółowy opis przedmiotu zamówienia zawierają załączniki: 1) Formularz cenowy (odrębnie dla każdego pakietu) - załącznik nr 1.1do SWZ, 2) Specyfikacja techniczna (odrębnie dla każdego pakietu) - załącznik nr 1.2 do SWZ, 3) Warunki gwarancji jakości i serwisu - załącznik nr 1.3. do SWZ, 4) Projektowane postanowienia umowy - załącznik nr 2 do SWZ. Zamawiający dopuszcza możliwość składania ofert częściowych, przedmiot zamówienia podzielony jest na 2 części (pakiety). Wykonawca może złożyć ofertę na jedną lub obie części zamówienia (pakiety). Zamawiający nie ogranicza liczby części zamówienia, którą można udzielić jednemu wykonawcy. Zamawiający informuje, iż zgodnie z art. 139 ustawy Pzp najpierw dokona badania i oceny ofert, a następnie kwalifikacji podmiotowej Wykonawcy, którego oferta została najwyżej oceniona w danym pakiecie, w zakresie braku podstaw do wykluczenia oraz spełniania warunków udziału w postępowaniu. Realizując obowiązek wynikający z art. 24 ust. 6 ustawy z dnia 14 czerwca 2024 r. o ochronie sygnalistów zamawiający informuje, że w Samodzielnym Publicznym Zakładzie Opieki Zdrowotnej Wojewódzkim Szpitalu Specjalistycznym Nr 3 w Rybniku przyjęta została procedura wewnętrznych zgłoszeń informacji o naruszeniach prawa i podejmowania działań następczych.
