Description

1. Przedmiotem zamówienia jest Dostawa jednorazowych wyrobów medycznych i higienicznych dla USK-2 PUM w Szczecinie 2. Ilekroć w dalszej części SWZ użyty będę zwrot „wyroby” należy rozumieć pod tym pojęciem wszelkie wyroby wskazane w załączniku nr 2 (FCJ) do SWZ. 3. Opis przedmiotu zamówienia został zawarty w formularzu cen jednostkowych (zał. 2 do SWZ, zwany dalej również jako „FCJ”), który winien być złożony wraz z formularzem oferty (zał. 1 do SWZ, zwany dalej również jako „FO”). 4. Zamawiający wymaga zaoferowania wyrobów medycznych - w rozumieniu Ustawy z dnia 7 kwietnia 2022 r. o wyrobach medycznych (Dz.U.2024.1620 z późn.zm.) wprowadzonych do obrotu i używania na terytorium RP zgodnie z obowiązującymi przepisami z wyłączeniem zadań 7, 8 (wyroby higieniczne). 5. Zamawiający wymaga, aby oferowane wyroby medyczne były dostosowane do przeprowadzenia skutecznego i bezpiecznego - zarówno dla pacjenta, jak i użytkownika - zabiegu medycznego, zgodnie z procedurą medyczną, w ramach której dany wyrób ma być stosowany. W szczególności wyroby muszą: być kompatybilne z techniką operacyjną stosowaną przez Zamawiającego, umożliwiać prawidłowe i precyzyjne wykonanie procedury medycznej, zapewniać bezpieczeństwo użytkowania, ergonomię oraz minimalizację ryzyka błędów i powikłań. Zamawiający wskazuje, iż zgodność z powyższym wymogiem zostanie potwierdzona przez członków Komisji Zamawiającego wspólnie z osobami, które na co dzień wykorzystują zaoferowane wyroby. Proces badania tychże wyrobów zostanie udokumentowany i załączony do protokołu niniejszego postępowania. 6. Ilości wyrobów podane w formularzu cen jednostkowych mają charakter orientacyjny i Zamawiający ma prawo zamówić ich mniejsze ilości, z tym, że nie mniej niż 60% wartości umowy/zadania. 7. Zamawiający na podstawie art. 30 ust. 2 ustawy PZP udziela zamówienia w częściach, z których każda stanowi przedmiot odrębnego postępowania. 8. Zamówienie składa się z 8 zadań. 9. Zamawiający wymaga by dostarczane wyroby posiadały okres przydatności do użycia minimum 24 miesiące licząc od daty dostawy do siedziby Zamawiającego. 10. Nie dopuszcza składania ofert wariantowych a także obejmujących tylko niektóre pozycje z zadania.
