Description

1. Przedmiotem zamówienia jest dostawa narzędzi chirurgicznych wielokrotnego użytku dla USK-2 w Szczecinie. 2. Ilekroć w dalszej części SWZ użyty będę zwrot „wyroby” - należy pod tym pojęciem rozumieć wyroby opisane w załączniku nr 2 do SWZ (formularz cen jednostkowych, zwany dalej FCJ ) oraz zał. nr 2A do SWZ ( formularz parametrów wymaganych, zwany dalej FPW ). 3. Ilości i szczegółowy opis wyrobów dla wszystkich zadań zawiera zał. nr 2 do SWZ (formularz cen jednostkowych).Dla zadań 1,2,3,4,5,8,9 szczególne wymagania Zamawiający opisał dodatkowo w zał. nr 2a do SWZ ( formularz parametrów wymaganych ). 4. Zamawiający wymaga zaoferowania wyrobów medycznych - w rozumieniu Ustawy z dnia 7 kwietnia 2022r o wyrobach medycznych (Dz.U. 2022 poz. 974 z późn. zm. ), dalej: „UoWM”), wprowadzonych do obrotu i używania na terytorium RP zgodnie z obowiązującymi przepisami. 5. Zamawiający wymaga by zaoferowane narzędzia chirurgiczne wielorazowego użytku były fabrycznie nowe, nieużywane, wyprodukowane nie wcześniej niż z 2026 roku. 6. Dla wszystkich zadań Zamawiający wymaga, by dostarczane wyroby posiadały okres gwarancji nie krótszy niż 24 miesiące ( nie dłuższy niż 48 miesięcy), licząc od daty ich dostarczenia do siedziby zamawiającego. Okres gwarancji stanowi jedno z kryterium oceny ofert. Dla zadań 1,2,3,4,5,8,9 Zamawiający opisał szczegółowo wymagania w tym zakresie w FPW. 7. Zamówienie składa się z 14 zadań. 8. Zamawiający na podstawie art. 30 ust. 2 ustawy PZP udziela zamówienia w częściach, z których każda stanowi przedmiot odrębnego postępowania. 9. Nie dopuszcza składania ofert wariantowych a także obejmujących tylko niektóre pozycje z zadania. 10.Zamawiający wymaga realizacji przedmiotu zamówienia przez okres: a) 12 miesięcy dla zadania 1-8; b) 3 miesięcy dla zadania 9-14. 11. Termin realizacji zamówienia częściowego wynosi: minimalnie 7 a maksymalnie 45 dni kalendarzowych , liczone od pierwszego dnia roboczego, przypadającego po dniu złożenia zamówienia telefonicznie lub e-mailem. Termin dostawy zamówienia cząstkowego stanowi jedno z kryterium oceny ofert.
