Zgodnie z Rozdziałem 8 SWZ Wykaz oświadczeń lub dokumentów potwierdzających spełnienie warunków udziału w postępowaniu, składanych przez Wykonawców zgodnie z art. 126 ust. 1 ustawy Pzp, po otwarciu ofert na wezwanie Zamawiającego: Aktualne zezwolenia na wytwarzanie lub import produktu leczniczego (w przypadku Wykonawcy mającego siedzibę lub miejsce zamieszkania poza terytorium Rzeczypospolitej Polskiej - dokument równoważny) lub kopia ważnego zezwolenia na prowadzenie hurtowni farmaceutycznych - wydanego przez Głównego Strona Inspektora Farmaceutycznego zgodnie z ustawą z dnia 06 września 2001 r. Prawo farmaceutyczne (t. j. Dz. U. z 2025 r. poz. 750). Informacje o wymaganych podmiotowych i przedmiotowych środkach dowodowych znajdują się w Rozdziale 5 i 8 Specyfikacji Warunków Zamówienia. Szczegółowy opis i zestawienie przedmiotu zamówienia stanowi załącznik nr 2 do SWZ.
