Opis przedmiotu zamówienia - formularz asortymentowo - cenowy - załącznik nr 1 do SWZ Pakiet nr 1 - Środki przeznaczone do stosowania jako roztwory do infuzji Pakiet nr 2 - Leki w postaci r-r do wstrzykiwań Pakiet nr 3 - Indygo karmin Pakiet nr 4 - Błękit metylenowy Pakiet nr 5 - Leki różne Pakiet nr 6 - Antybiotyki cz.1 Pakiet nr 7 - Antybiotyki cz.2 5.3.2. Projektowane postanowienia umowy - załącznik nr 2 do SWZ. 5.4. Formularz asortymentowo - cenowy stanowi treść oferty i nie podlega uzupełnieniu.
