Social Care Wales is seeking a supplier to design and develop a coherent, evidence-informed and implementation ready Multi-Disciplinary Team (MDT) support offer for local authorities across Wales. Through stakeholder engagement, co-design, targeted review of existing provision, testing and feasibility assessment, the successful Supplier will develop a practical, scalable and sustainable support model that complements existing improvement activity, addresses current system challenges, and provides a clear implementation pathway for national rollout. The commission is focused on the development, refinement and validation of the MDT Support Offer and is not intended to undertake extensive national mapping exercises, options appraisals or delivery of the future service.
