Pohjois-Karjalan hyvinvointi alue hakee 3T MRI laitetta Joensuun keskussairaalaan, Kuvantamiskeskukseen/radiologiaan. Sairaalamme sijaitsee Joensuussa ja palvelee laajasti koko Pohjois Karjalan aluetta. Kuvantamisyksikössämme on tällä hetkellä käytössä kaksi Siemensin Sola 1,5T MRI laitetta, joilla kuvataan kattavasti kaikkia potilasryhmiä vauvasta ikääntyneisiin potilaisiin. Toimintamme kattaa myös vaativat potilasryhmät, mukaan lukien anestesiassa kuvattavat sekä implanttipotilaat. Kuvaamme laajasti eri anatomisia alueita, mutta tällä hetkellä emme tee MRI ohjattuja toimenpiteitä emmekä tutkimustoimintaa. Lisäksi Kuvantamiskeskuksessa leikekuvantamislaitteiden osalta käytössämme on kolme Siemensin tietokonetomografialaitetta sekä Siemensin SPECT- ja PET-järjestelmät. Radiologimme ja muut kuvantamiseen perehtyneet erikoislääkärit käyttävät lausuntotyössä Siemensin syngo.via työasemaratkaisua, jota hyödynnetään laajasti kaikissa leikekuvantamismodaliteeteissa, erityisesti TT- ja MRI kuvantamisessa, sekä soveltuvin osin myös SPECT ja PET tutkimuksissa. Hankittava uusi 3T MRI laite tulee olemaan sairaalamme ensimmäinen ja ainoa 3T tason järjestelmä. Laitetta tullaan alkuvaiheessa käyttämään erityisesti neuro-, tuki ja liikuntaelimistön (MSK), rintojen sekä eturauhasen kuvantamiseen. Lisäksi laitteella on tarkoitus suorittaa sydänkuvantamista, mukaan lukien kohdennetut tutkimukset iskeemisten muutosten diagnosointiin, sepelvaltimoiden anatomian arviointiin sekä sydänlihaksen perfuusion kuvantamiseen levossa ja rasituksessa. Tavoitteenamme on jatkossa laajentaa 3T tutkimusten käyttöä kattavasti, minkä vuoksi laitteen monipuolinen varustelu ja laajennettavuus ovat meille erittäin tärkeitä. Sairaalamme sijainti huomioiden arvostamme luotettavan, nopeasti saatavilla olevan ja kokeneen huollon merkitystä. Lisäksi koulutuksella on keskeinen rooli, koska kuvaustoimintaan osallistuu useita hoitajia. Edellytämme, että käyttökoulutus on saatavilla suomen kielellä. Myös huollon suomenkielinen palvelu katsotaan merkittäväksi väärinymmärrysten välttämiseksi. MRI järjestelmän käyttöliittymän yhtenäisyydellä ja käytettävyydellä on keskeinen merkitys yksikkömme toiminnan, koulutuksen ja teknisen ylläpidon kannalta. Yksikössämme henkilöstöresurssit ovat rajalliset, ja kuvaustoimintaan osallistuu useita käyttäjiä, minkä vuoksi uuden järjestelmän käyttöönoton ja koulutuksen tulee olla mahdollisimman sujuvaa ja tehokasta. Useiden erilaisten käyttöliittymien samanaikainen käyttö lisää merkittävästi koulutustarvetta, monimutkaistaa työnkulkua ja kasvattaa virheiden riskiä. Lisäksi henkilöstön irrottaminen kliinisestä potilastyöstä koulutukseen on haastavaa, mikä korostaa selkeän, intuitiivisen ja mahdollisimman yhdenmukaisen käyttöliittymän merkitystä. Vastaavasti yhtenäinen käyttöympäristö helpottaa myös IT ylläpitoa, päivityksiä ja tukipalveluita sekä parantaa järjestelmän kokonaisluotettavuutta. Tämän vuoksi pidämme tärkeänä, että tarjottu ratkaisu tukee mahdollisimman sujuvaa käyttöönottoa, tehokasta koulutusta ja hallittavaa ylläpitoa yksikön resurssit huomioiden. Ympäristövastuu on keskeinen arvo toiminnassamme. Edellytämme, että tarjottu MRI järjestelmä on valmistettu vastuullisesti ja sen ympäristökuormitus on mahdollisimman vähäinen. Hyväksymme myös tehdaskunnostetun laitteen, mikäli se täyttää vaaditut tekniset ja toiminnalliset kriteerit. Hankinta toteutetaan hankinnalle osoitettujen määrärahojen puitteissa. Hankintayksikkö pidättää oikeuden keskeyttää hankintamenettelyn, mikäli hyväksyttävissä olevien tarjousten hinnat ylittävät hankinnalle käytettävissä olevan määrärahan eikä hankintaa voida toteuttaa käytettävissä olevalla rahoituksella.
