Wykonawca zobowiązany jest zrealizować zamówienie na zasadach i warunkach określonych we Wzorze Umowy stanowiącym Załącznik nr 3 do SWZ, Opisie przedmiotu zamówienia - stanowiącym załącznik nr 1 do SWZ, Formularzu oferty, stanowiącym załącznik nr 2 do SWZ oraz pozostałych załącznikach do SWZ 3. Załącznik nr 1 do SWZ - Opis przedmiotu zamówienia, Wykonawca zobowiązany jest wypełnić i podpisany załączyć do oferty.
