Description

1. Przedmiotem zamówienia jest usługa serwisowa sprzętu medycznego będącego własnością Zamawiającego, zainstalowanego w Szpitalu Miejskim w Rudzie Śląskiej Sp. z o.o. Szczegółowy opis przedmiotu zamówienia w zakresie świadczenia usług został określony w załączniku nr 1 do SWZ/UMOWY, w załączniku nr 6 do SWZ (dotyczy pakietu nr 1), w załączniku nr 7 do SWZ (dotyczy pakietu nr 2), w załączniku nr 8 do SWZ (dotyczy pakietu nr 3), w załączniku nr 9 do SWZ (dotyczy pakietu nr 4). 2. Przedmiot zamówienia został podzielony na 4 części (pakiety). 4. Postępowanie prowadzone jest w trybie przetargu nieograniczonego, zgodnie z ustawą z dnia 11 września 2019 r. Prawo zamówień publicznych (t.j. Dz. U. z 2024 r. poz. 1320 z późn. zm.). 5. Zgodnie z art. 95 ust. 1 ustawy Zamawiający wymaga, aby osoby wykonujące czynności bezpośrednio związane z wykonywaniem usługi serwisowej były zatrudnione na umowę o pracę. 6. Wymagania dotyczące sposobu dokumentowania zatrudnienia osoby na umowę o pracę, uprawnienia kontrolne Zamawiającego oraz sankcje z tytułu niespełnienia wymagań o którym mowa w pkt 5 powyżej, zostały szczegółowo określone w projektowanych postanowieniach umowy, które zostaną wprowadzone do treści umowy w sprawie zamówienia (załącznik nr 6, 7, 8, 9 do SWZ). Usługa serwisowa dotyczy sprzętu medycznego firmy Siemens (tomograf komputerowy,serwer syngo.via). Szczegółowy opis przedmiotu zamówienia zawiera załącznik nr 1 do SWZ/UMOWY oraz załącznik nr 6 do SWZ.; Pakiet nr 2 - Usługa serwisowa sprzętu medycznego firmy Puritan Bennett, Newport - Respiratory. Szczegółowy opis przedmiotu zamówienia zawiera załącznik nr 1 do SWZ/UMOWY oraz załącznik nr 7 do SWZ.; Pakiet nr 3 - Usługa serwisowa sprzętu medycznego firmy Philips (Aparat RTG). Szczegółowy opis przedmiotu zamówienia zawiera załącznik nr 1 do SWZ/UMOWY oraz w załączniku nr 8 do SWZ.; Pakiet nr 4 - Usługa serwisowa dotyczy sprzętu medycznego firmy SMS - (sterylizatory). Szczegółowy opis przedmiotu zamówienia zawiera załącznik nr 1 do SWZ/UMOWY oraz załącznik nr 9 do SWZ.
