1. Przedmiotem zamówienia jest: Kompleksowe ubezpieczenie Samodzielnego Publicznego Zakładu Opieki Zdrowotnej w Przeworsku” 2. Rodzaj zamówienia: usługa. 3. Przedmiot zamówienia sklasyfikowany jest we Wspólnym Słowniku Zamówień kod CPV: 66510000-8 - usługi ubezpieczeniowe, 66515000-3 - usługi ubezpieczenia od uszkodzenia lub utraty, 66516000-0 - usługi ubezpieczenia od odpowiedzialności cywilnej, 66516100-1 - usługi ubezpieczenia pojazdów mechanicznych od odpowiedzialności cywilnej, 66514110-0 - usługi ubezpieczeń pojazdów mechanicznych, 66512100-3 usługi ubezpieczenia od następstw nieszczęśliwych wypadków. 4. Przedmiot zamówienia został podzielony na części. Ubezpieczenie obejmuje: Część I: Ubezpieczenie mienia a) Ubezpieczenie mienia od wszystkich ryzyk Część II: Ubezpieczenia OC a) Obowiązkowe ubezpieczenie odpowiedzialności cywilnej podmiotu wykonującego działalność leczniczą, b) Dobrowolne ubezpieczenie odpowiedzialności cywilnej za szkody wyrządzone osobie trzeciej w następstwie udzielania świadczeń zdrowotnych albo niezgodnego z prawem zaniechania udzielania świadczeń zdrowotnych w związku z wykonywaniem działalności leczniczej, c) Dobrowolne ubezpieczenie odpowiedzialności cywilnej za szkody wyrządzone osobie trzeciej z tytułu prowadzonej działalności i posiadanego mienia. Część III: Ubezpieczenia komunikacyjne a) Obowiązkowe ubezpieczenie odpowiedzialności cywilnej posiadaczy pojazdów mechanicznych (OC ppm.), b) Ubezpieczenie auto casco (AC+ASS), c) Ubezpieczenie następstw nieszczęśliwych wypadków kierowcy i pasażerów (NNW), 5. Szczegółowe informacje na temat przedmiotu zamówienia znajdują się w Załączniku nr 1; 6; 6a; 7 do SWZ - Opis przedmiotu zamówienia) oraz w załącznikach do SWZ/OPZ. 6. Zamawiający nie przewiduje możliwości zastosowania prawa opcji. 7. Zamawiający przewiduje możliwość udzielenia zamówień, o których mowa w art. 214 ust. 1 pkt 7) pzp. w wysokości 15% dla ubezpieczeń mienia część I. a) Ubezpieczenie mienia od wszystkich ryzyk; a) Obowiązkowe ubezpieczenie odpowiedzialności cywilnej podmiotu wykonującego działalność leczniczą, b) Dobrowolne ubezpieczenie odpowiedzialności cywilnej za szkody wyrządzone osobie trzeciej w następstwie udzielania świadczeń zdrowotnych albo niezgodnego z prawem zaniechania udzielania świadczeń zdrowotnych w związku z wykonywaniem działalności leczniczej, c) Dobrowolne ubezpieczenie odpowiedzialności cywilnej za szkody wyrządzone osobie trzeciej z tytułu prowadzonej działalności i posiadanego mienia.; a) Obowiązkowe ubezpieczenie odpowiedzialności cywilnej posiadaczy pojazdów mechanicznych (OC ppm.), b) Ubezpieczenie auto casco (AC+ASS), c) Ubezpieczenie następstw nieszczęśliwych wypadków kierowcy i pasażerów (NNW)
