1. Przedmiotem zamówienia jest Usługi medyczne obejmujące profilaktyczne badania lekarskie(wstępne, okresowe, kontrolne, sanitarno - epidemiologiczne, badania diagnostyczne, badania laboratoryjne, badania psychotechniczne, konsultacje specjalistyczne), żołnierzy zawodowych i pracowników resortu obrony narodowej 43 Wojskowego Oddziału Gospodarczego oraz żołnierzy zawodowych i pracowników JW. i instytucji będących na zaopatrzeniu 43WOG. 2. Zamawiający informuje, że przewiduje korzystanie z zamówień opcjonalnych (prawo opcji), zgodnie z zapisami wzoru umowy stanowiącej Załącznik nr 8 do SWZ. Zamawiający w załączniku nr 6a-6c do SWZ (formularze cenowe) określa podstawowy poziom zamówienia, który może poszerzyć poprzez zwiększenie ilości dla: Części 1 do 150% wartości zamówienia podstawowego. Części 2 do 150% wartości zamówienia podstawowego. Części 3 do 100% wartości zamówienia podstawowego. 3. Szczegółowy opis przedmiotu zamówienia znajduje się w załączniku nr 9 do SWZ. Usługi medyczne dla żołnierzy i pracowników resortu obrony narodowej rejonu Żagań, Świętoszów, Potok. Wykonawca realizujący przedmiot zamówienia musi dysponować gabinetem lekarskim medycyny pracy oraz bazą do wykonywania badań laboratoryjnych i innych określonych w załączniku Nr 1 do SWZ w odległości do 50km od miejsca stacjonowania Jednostek Wojskowych i Instytucji.; Usługi medyczne dla żołnierzy i pracowników resortu obrony narodowej rejonu Bolesławiec.
