Description

1. Przedmiotem zamówienia jest „Usługa przeprowadzenia szkoleń i powiązanych form wsparcia w ramach projektu „Opieka koordynowana w Podstawowej Opiece Zdrowotnej” 2. Przedmiot zamówienia realizowany jest związku z projektem: „Opieka koordynowana w POZ” w ramach projektu Fundusze Europejskie dla Rozwoju Społecznego 2021-2027 współfinansowanego ze środków Europejskiego Funduszu Społecznego Plus” w ramach umowy: FERS.04.15-IP. P.07-0003/23-00/1374/2024/217). 3. Przedmiot zamówienia obejmuje przeprowadzenie: 1) analizy i oceny wdrożenia rozwiązań w zakresie koordynacji opieki w POZ; 2) szkoleń w formie hybrydowej oraz online z tematyki opieki koordynowanej, diagnostyki, leczenia oraz profilaktyki; 3) dwudniowych wizyt szkoleniowych na miejscu w podmiocie POZ poświęconych praktycznym aspektom wdrażania opieki koordynowanej; 4) coachingu po zakończeniu szkoleń. 3. Zamawiający dodatkowo przewiduje wykorzystanie Prawa Opcji, polegającego na przeprowadzeniu do 30 dodatkowych szkoleń dla uczestników szkolenia, na zasadach tożsamych z opisem przedmiotu zamówienia dla zamówienia podstawowego. 1) Opcja zostanie uruchomiona pod warunkiem, że wartość zamówienia podstawowego wraz z opcją będzie mieściła się w wartości szacunkowej przeznaczonej na realizację zamówienia przez Zamawiającego 2) W celu skorzystania z Prawa Opcji, Zamawiający składa Wykonawcy pisemne oświadczenie o wykonaniu Prawa Opcji, nie później niż w terminie jednego miesiąca przed terminem zakończenia obowiązywania Umowy. Opcja zostanie uruchomiona na warunkach określonych w umowie. 4. Opis przedmiotu zamówienia zawiera Załącznik nr 1 do Specyfikacji. 5. Sposób realizacji zamówienia określa projekt umowy stanowiący Załącznik nr 2 do Specyfikacji. 6. Zamawiający nie przewiduje udzielenie zamówień na podstawie art. 305 w związku z art. 214 ust. 1 pkt 7 i 8 ustawy Pzp. 5.
