1. Przedmiotem zamówienia jest dostawa sprzętu medycznego do Miejskiego Centrum Medycznego im. dr. Karola Jonschera w Łodzi w podziale na 27 pakietów. Zamawiający dopuszcza możliwość składania ofert częściowych w podziale na 27 Pakietów. Zamawiający nie dopuszcza możliwości składania oferty na wybrane pozycje w ramach Pakietów (Pakiety są niepodzielne). Szczegółowy opis przedmiotu zamówienia określa załącznik nr 10 do SWZ. Dla Pakietów 1,2,3,4,6,7,9,10,14,17,21 - Zamawiający wymaga w zakresie dostawy: instalacji, uruchomienia oraz szkolenia personelu 2. Dostawa przedmiotu zamówienia do Miejskiego Centrum Medycznego im. dr. Karola Jonschera w Łodzi (w lokalizacji przy ul. Milionowej 14, 93-113 Łódź) zrealizowana będzie na koszt i ryzyko Wykonawcy, dostawa dla pakietu 15 - w lokalizacji Łódź, ul. Przyrodnicza 7/9. 3. Warunki realizacji przedmiotu zamówienia określa załącznik nr 6 do SWZ (Wzór umowy). Zakres I - 1x System ucyfrowienia bezpośredniego aparatów RTG trzema detektorami bezprzewodowymi, stacją dokującą do detektorów - szt. 1, stacjonarną konsolą technika - 1szt.; Zakres II - Aparat do angiografii fluoresceinowej siatkówki; Zakres III - Lampa szczelinowa, cyfrowa; Zakres IV - Laser Yag ze stolikiem okulistycznym; Zakres V Narzędzia chirurgiczne ortopedyczne; Zakres VI - Defibrylator; Zakres VII - Stół operacyjny ortopedyczny z przystawkami ortopedycznymi; Zakres VIII Przenośny zestaw rektoskopowy z głowicą wielorazową i tubusami jednorazowymi; Zakres IX - Tonometr przenośny do pomiaru cisnienia wewnatrzgałkowego oka; Zakres X - Aparat ultrasonograficzny z głowicą ortopedyczną; Zakres XI - Regał uniwersalny do przechowywania implantów i instrumentów, Taboret medyczny bez oparcia, na kółkach; Zakres XII - Tonometr Schiotza; Zakres XIII - Taboret medyczny na kółkach z oparciem; Zakres XIV -Aparat EKG 12-kanałowy; Zakres XV - Wózek toaletowo-prysznicowy; Zakres XVII - System do ogrzewania pacjenta ciepłym powietrzem; Zakres XVIII - Taboret medyczny na kółkach z oparciem; Zakres XIX Wózek oddziałowy; Zakres XX - Wózek reanimacyjny; XXI Tonometr aplanacyjny; XXII - Test diagnostyczny do oceny widzenia stereoskopowego; XXIII - Soczewka gonioskopowa typu szerokokatnego , Soczewka gonioskopowa; XXIV- Narzędzia chirurgiczne okulistyczne; Zakres XXV - Lampa bakteriobójcza; Zakres XXVI -Materace przeciwodleżynowe; Zakres XXVII -Wózek do przewozu chorych
