Zamawiający informuje, że zamówienie będzie dofinansowane w ramach Krajowego Planu Odbudowy i Zwiększania Odporności: Komponent D „Efektywność, dostępność i jakość systemu ochrony zdrowia” w zakresie inwestycji D1.1.1 „Rozwój i modernizacja infrastruktury centrów opieki wysokospecjalistycznej i innych podmiotów leczniczych”. Przedsięwzięcie będzie realizowane zgodnie z zasadą DNSH („do no significant harm”). Wykonawca zobowiązany jest podczas realizacji zamówienia do działania zgodnie z zasadą równości szans i niedyskryminacji, w tym równości kobiet i mężczyzn. Wykonawca zobowiązuje się: 1. Dostarczyć sprzęt z instrukcją obsługi do siedziby Zamawiającego w terminie do dnia 15 czerwca 2026 r. wraz z podpisaniem protokołu odbioru końcowego i wystawieniem faktury. Szkolenie personelu powinno nastąpić w terminie do dnia 25 czerwca 2026 roku - zapis dotyczy zakresu nr 1-5. 2. Dostarczyć sprzęt z instrukcją obsługi do siedziby Zamawiającego, zainstalować go, zamontować, uruchomić w nieprzekraczalnym terminie do dnia 15 czerwca 2026 r. wraz z podpisaniem protokołu odbioru końcowego i wystawieniem faktury. Szkolenie personelu powinno nastąpić w terminie do dnia 25 czerwca 2026 roku - zapis dotyczy zakresu nr 6,7,8. 3. Dostarczyć sprzęt z instrukcją obsługi do siedziby Zamawiającego w terminie do dnia 15 czerwca 2026 r. wraz z podpisaniem protokołu odbioru końcowego i wystawieniem faktury - zapis dotyczy zakresu nr 9-13. 4. Termin rozpoczęcia instalacji sprzętu będzie uzależniony od postępu prowadzenia prac wykończeniowych obiektu. 5. W ramach przedmiot postepowania należy: a) zainstalowanie, montaż i uruchomienie urządzeń - dotyczy zakresów nr 6,7 ,8 b) dostarczenie instrukcji obsługi w języku polskim w wersji papierowej i elektronicznej,(zakres nr 1-13) c) szkolenie przez Wykonawcę personelu Szpitala - dotyczy zakresów nr 1 do 8 d) uzyskanie pozytywnej decyzji UDT - zapis dotyczy zakresu nr 7, 6. Cena będzie płatna w następujący sposób: a) 70% ceny w terminie do dnia 31 marca 2026 r. w formie zaliczki na poczet wykonania zamówienia na podstawie wystawionej przez Wykonawcę faktury zaliczkowej, b) 30% ceny w terminie do 30 czerwca 2026 r. po podpisaniu protokołu odbioru końcowego. Jeżeli Wykonawca wykona wszystkie obowiązki umowne i strony podpiszą protokół odbioru końcowego przed dniem 30 marca 2026 roku Zamawiający zapłaci całość ceny w terminie do dnia 31 marca 2026 roku.
