Przedmiotem zamówienia jest dostawa sprzętu i wyposażenia medycznego realizowanego w ramach projektu: ,, Rozwój Ambulatoryjnej Opieki Specjalistycznej’’ z podziałem na 8 zadań opisanych w załącznikach pn. Parametry techniczne ; 1) Wyposażenie gabinetu medycznego 2) Aparaty USG 3) Wyposażenie gabinetu okulistycznego 4) Wyposażenie gabinetu fizjoterapii 5) Wyposażenie gabinetu poradni K 6) Aparat EKG 7) Sprzęt IT 8) Aparat do kriochirurgii 2. Zamawiający dokonuje podziału zamówienia na zadania 3. Wykonawca winien uwzględnić w cenie oferty wszystkie przewidywane koszty realizacji zamówienia, które będą miały wpływ na cenę oferty. a) dostawa, montaż, uruchomienie b) szkolenie personelu obsługującego aparaturę( sprzęt medyczny) c) koszty gwarancji (tj. nieodpłatne przeglądy, naprawy, materiały i części wymienne) 1) Wyposażenie gabinetu medycznego; 2) Aparaty USG; 3) Wyposażenie gabinetu okulistycznego; 4) Wyposażenie gabinetu fizjoterapii; 5) Wyposażenie gabinetu poradni K; 6) Aparat EKG; 7) Sprzęt IT; 8) Aparat do kriochirurgii
