Przedmiotem niniejszej umowy jest dostawa, wniesienie, rozmieszczenie, montaż, instalacja oraz uruchomienie angiografu wraz z wyposażeniem, zwanego dalej „przedmiotem zamówienia” lub „sprzętem” w Pracowni Hemodynamiki (dotyczy części nr 1) / w Pracowni Elektrofizjologii (dotyczy części nr 2) Wojewódzkiego Szpitala Specjalistycznego im. Janusza Korczaka w Słupsku Sp. z o.o., przy ul. Hubalczyków 1, zgodnie z Formularzem asortymentowo-ilościowym oraz Opisem Przedmiotu Zamówienia. Zakres przedmiotowy został szczegółowo opisany w załączeniu do niniejszej SWZ oraz we wzorze umowy- Część II SWZ. Opis przedmiotu zamówienia zawiera zestawienie parametrów technicznych, warunków gwarancji oraz szkoleń - Część III SWZ. Opis przedmiotu zamówienia należy odczytywać wraz z ewentualnymi zmianami treści specyfikacji, będącymi np. wynikiem udzielonych odpowiedzi na zapytania wykonawców. Przedmiotem niniejszej umowy jest dostawa, wniesienie, rozmieszczenie, montaż, instalacja oraz uruchomienie angiografu wraz z wyposażeniem, zwanego dalej „przedmiotem zamówienia” lub „sprzętem” w Pracowni Hemodynamiki (dotyczy części nr 1) Wojewódzkiego Szpitala Specjalistycznego im. Janusza Korczaka w Słupsku Sp. z o.o., przy ul. Hubalczyków 1, zgodnie z Formularzem asortymentowo-ilościowym oraz Opisem Przedmiotu Zamówienia.
