Description

Przedmiotem zamówienia jest: a. Zestawy do ciągłych technik nerkozastępczych dla Oddziału Pooperacyjnego CMN i Oddziału Transplantacji Płuc - w ilości i wymaganiach określonych w Załączniku nr 3 do SWZ. b. Dzierżawa aparatów do ciągłych technik nerkozastępczych - 9 sztuk dla Kliniki Anestezjologii i Intensywnej Terapii UCK (kompatybilnych z zestawami do ciągłych technik nerkozastępczych opisanych jako pakiet 1 w załączniku nr 3 do SWZ), zgodnie z opisem zawartym w Załączniku nr 4a do SWZ. c. Dzierżawa aparatów do ciągłych technik nerkozastępczych - 2 sztuk dla Kliniki Kardiochirurgii - Oddziału Transplantologii Płuc UCK (kompatybilnych z zestawami do ciągłych technik nerkozastępczych opisanymi jako pakiet 2 w załączniku nr 3 do SWZ oraz z płynami substytucyjnymi/dializacyjnymi, antykoagulacyjnymi oraz roztworem chlorku wapnia do ciągłych terapii opisanymi w załączniku nr 5 do SWZ), zgodnie z opisem zawartym w Załączniku nr 4b do SWZ. d. Dostawa płynów substytucyjnych/dializacyjnych, antykoagulacyjnych oraz roztworu chlorku wapnia do ciągłych terapii w ilości i wymaganiach określonych w Załączniku nr 5 do SWZ. 2. Zamawiający dopuszcza możliwość składania ofert częściowych w ramach dwóch Pakietów. Pod pojęciem oferty częściowej rozumie się pojedyncze pakiety ustalone poniżej: Pakiet 1 - Zestawy do ciągłych technik nerkozastępczych (Pakiet 1, Załącznik nr 3 do SWZ) wraz z dzierżawą aparatów do ciągłych technik nerkozastępczych - 9 sztuk (Załącznik nr 4a do SWZ) Pakiet 2 - Zestawy do ciągłych technik nerkozastępczych (Pakiet 2, Załącznik nr 3 do SWZ) oraz asortyment opisany w załączniku nr 5 do SWZ wraz z dzierżawą aparatów do ciągłych technik nerkozastępczych - 2 sztuki (Załącznik nr 4b do SWZ) Wykonawca może złożyć ofertę na jedną lub obie części.
