Description

Przedmiotem zamówienia jest sukcesywna dostawa leków w ramach RDTL w ciągu 12 miesięcy Postępowanie składa się z 3 części 3.2. Szczegółowy opis przedmiotu zamówienia, wymagana postać, pojemność/objętość, dawka, jednostka miary, ilość w opakowaniu, ilość opakowań określa FORMULARZ ASORTYMENTOWO-CENOWY, stanowiący załącznik nr 1 do SWZ. 3.3 Szczegółowe warunki i zasady realizacji przedmiotu zamówienia określają dodatkowo warunki umowne będące załącznikiem nr 5 do SWZ „Warunki umowne”. 3.4. Termin dostawy - wg bieżących potrzeb, każdorazowo w terminie maksymalnie 2 dni roboczych od daty złożenia zamówienia. 3.5. Seria i data ważności na opakowaniu jednostkowym oraz na fakturze. 3.6. Termin ważności oferowanych produktów nie krótszy niż 12 miesięcy liczony od daty dostawy do siedziby Zamawiającego. 3.7. Wykonawca dostarczy przedmioty zamówienia do Magazynu APTEKI SZPITALA. 3.8. Zamawiający żąda wskazania przez wykonawcę w ofercie części zamówienia, których wykonanie zamierza powierzyć podwykonawcom i podania przez Wykonawcę firm podwykonawców - zgodnie z art. 462 ust. 2 ustawy PZP, jeżeli są już znani. Przedmiotowe środki dowodowe Kserokopie dokumentów potwierdzających dopuszczenie do obrotu oferowanego produktu zgodnie z ustawą Prawo Farmaceutyczne - jeżeli dotyczy; „OŚWIADCZENIE, że Wykonawca posiada aktualne dokumenty oferowanego produktu farmaceutycznego dopuszczające do obrotu na wymaganą postać i dawkę leku, i stosowania w ochronie zdrowia na terytorium Rzeczypospolitej Polskiej, zgodnie z polskim prawem oraz prawem Unii Europejskiej oraz Charakterystykę Produktu Leczniczego (CHPL) oferowanego leku wraz z oświadczeniem, że w/w dokumenty zostaną udostępnione na każde żądanie Zamawiającego”.
