Przedmiotem zamówienia jest dostawa produktów do żywienia dojelitowego, pozajelitowego oraz wyrobów medycznych do podaży diet w ilościach i asortymencie wskazanych w Formularzach asortymentowo - cenowych zawartych w Załączniku nr 2 do SWZ. Dostawy będą następować do miejsca odbioru: Apteki Szpitalnej w Będzinie przy ul. Małachowskiego 12 lub do pomieszczeń wskazanych przez Zamawiającego w Czeladzi, ul. Szpitalna 40 przez okres 12 miesięcy od pierwszego dnia obowiązywania umowy.
