Opis przedmiotu zamówienia 1. Przedmiotem zamówienia dostawa leków różnych i płynów infuzyjnych oraz leków programowych Szpitala Wojewódzkiego im. Jana Pawła II w Bełchatowie. 2. Kod i nazwa według Wspólnego Słownika Zamówień (CPV): 33.60.00.00-6 Produkty farmaceutyczne 3. Opis, wielkość i zakres przedmiotu zamówienia: 1) Zamawiający podzielił przedmiot zamówienia na 56 części (Pakiety nr 1 - 56), zgodnie z opisem zamieszczonym w Rozdziale XXIII SWZ. 2) Pozostałe wymagania dotyczące realizacji zamówienia oraz zasady współpracy między zamawiającym a wykonawcą określają projektowane postanowienia umowy w sprawie zamówienia publicznego, stanowiące Załącznik nr 3 do SWZ. 3) Jeżeli w opisie przedmiotu zamówienia znajdują się wskazania znaków towarowych, patentów lub pochodzenia, źródła lub szczególnego procesu, który charakteryzuje produkty dostarczane przez konkretnego wykonawcę, należy przyjąć, że wskazaniu takiemu towarzyszą wyrazy „lub równoważny”. W takim przypadku zamawiający dopuszcza możliwość złożenia przez wykonawcę oferty na preparat równoważny, przy czym oferta taka polega na zaoferowaniu preparatów tożsamych z przedmiotem zamówienia, dla których istnieje wspólna nazwa wg indeksu nazw międzynarodowych. W przypadku wątpliwości zamawiającego dotyczących równoważności zaoferowanych leków, wykonawca zobowiązany będzie do złożenia wyjaśnień i wykazania ich równoważności na każde wezwanie zamawiającego. Nie dopuszcza się składania ofert na preparaty zarejestrowane jako „suplement diety”.
