Description

1. Przedmiotem zamówienia jest dostawa preparatów do żywienia dojelitowego dla UCK w asortymencie, szacunkowej ilości oraz wymaganiach określonych w załączniku nr 3 do SWZ. 2. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze pozycje ustalone w załączniku nr 3 do SWZ. Wykonawca może złożyć ofertę na jedną lub kilka pozycji. 3. Dostawy będą następować sukcesywnie, w ilości i asortymencie, zgodnie z zamówieniami częściowymi Zamawiającego w terminie do 2 dni od daty złożenia zamówienia. Zamawiany asortyment Wykonawca zobowiązany będzie dostarczyć na własny koszt do Apteki Szpitalnej. 4. W przypadku, gdy Zamawiający użył w opisie przedmiotu zamówienia oznaczeń norm, ocen technicznych, specyfikacji technicznych i systemów referencji technicznych, o których mowa w art. 101 ust. 1 pkt 2 oraz ust. 3 ustawy Pzp, dopuszcza się rozwiązania równoważne. Każdorazowo, gdy wskazana jest w niniejszej SWZ lub załącznikach do SWZ norma, należy przyjąć, że w odniesieniu do niej użyto sformułowania „lub równoważna”. W przypadku gdy opis przedmiotu zamówienia odnosi się do norm, ocen technicznych, specyfikacji technicznych i systemów referencji technicznych, o których mowa w ust. 1 pkt 2 oraz ust. 3, zamawiający nie może odrzucić oferty tylko dlatego, że oferowane dostawy nie są zgodne z normami, ocenami technicznymi, specyfikacjami technicznymi i systemami referencji technicznych, do których opis przedmiotu zamówienia się odnosi, pod warunkiem że wykonawca udowodni w ofercie, w szczególności za pomocą przedmiotowych środków dowodowych, o których mowa w art. 104-107, że proponowane rozwiązania w równoważnym stopniu spełniają wymagania określone w opisie przedmiotu zamówienia. 5. Zamawiający wymaga złożenia ofert w postaci elektronicznej. Szczegółowy opis (instrukcja) składania ofert w postaci elektronicznej zawarty został w ust XIV SWZ. Zamawiający informuje, iż w przedmiotowym postępowaniu o udzielenie zamówienia publicznego zastosowana zostanie tzw. „odwrócona kolejność oceny ofert”, o której mowa w art. 139 ustawy PZP. Zamawiający stosując powyższą procedurę, w pierwszej kolejności dokona badania i oceny ofert, a następnie dokona kwalifikacji podmiotowej wykonawcy, którego oferta została najwyżej oceniona w zakresie braku podstaw do wykluczenia oraz spełniania warunków udziału w postepowaniu. 14. Wykonawca może powierzyć wykonanie części zamówienia podwykonawcy. 15. Zamawiający nie dopuszcza składania ofert wariantowych. 16. Kod Wspólnego Słownika Zamówień (CPV): zgodnie z załącznikiem nr 3 do SWZ.
