Przedmiotem zamówienia jest Dostawa odczynników laboratoryjnych wraz z dzierżawą analizatorów dla Szpitala Powiatowego w Chrzanowie - 9 pakietów. 3. Dokumenty stanowiące ofertę, które należy złożyć: a) Formularz ofertowy (zał. 1), b) Kosztorys szczegółowy/wyceniony pakiet/y (zał. 2) wraz z wypełnionym i podpisanym Arkuszem Wymagań do Pakietów (zał. 8 - jeśli dotyczy) c) Jednolity Europejski Dokument Zamówienia - w przypadku wspólnego ubiegania się o zamówienie przez Wykonawców, oświadczenie o niepoleganiu wykluczeniu składa każdy z Wykonawców (zał. 3), d) Oświadczenie Wykonawcy (zał. nr 6), że nie podlega wykluczeniu na podstawie: - art. 7 ust. 1 ustawy z dnia 13 kwietnia 2022r. o szczególnych rozwiązaniach w zakresie przeciwdziałania wspieraniu agresji na Ukrainę oraz służących ochronie bezpieczeństwa narodowego, - art. 5k rozporządzenia 833/2014 w brzmieniu nadanym rozporządzeniem 2022/576 z późn.zm. e) Pełnomocnictwo upoważniające do złożenia oferty, o ile ofertę składa pełnomocnik; f) Pełnomocnictwo dla pełnomocnika do reprezentowania w postępowaniu Wykonawców wspólnie ubiegających się o udzielenie zamówienia - dotyczy ofert składanych przez Wykonawców wspólnie ubiegających się o udzielenie zamówienia; Aparat do szybkiej identyfikacji wykorzystujący technologię spektrometrii masowej MALDI-TOF w zakresie badań mikrobiologicznych; Testy do aparatu VITEK wraz z dzierżawą aparatu backup; Testy immunochromatograficzne, wymazówki; Oznaczanie lekowrażliwości - krążki antybiotykowe, E-testy; Odczynniki do aparatu MAGLUMI; Odczynniki do aparatu EuroBlot One; Odczynniki do RKZ z hemoglobiną tlenko-węglową wraz z dzierżawą aparatu Oddział Położnictwa; Zestawy do hodowli bakterii beztlenowych; Odczynniki do aparatu SIMPLEX
