PCK, ul. Postania Styczniowego 1, 81-519 Gdynia b) Szpital Św. Wincentego a Paulo, ul. Wójta Radtkego 1, 81-348 Gdynia c) Centrum Medyczne Smoluchowskiego - Gdańsk, ul. Smoluchowskiego 18, 80-214 Gdańsk d) Szpital Specjalistyczny im. F. Ceynowy, ul. dr A. Jagalskiego 10, 84-200 Wejherowo 2. Szczegółowy opis przedmiotu zamówienia, asortyment i szacunkowe ilości podano w Formularzu asortymentowo-cenowym (załączniku nr 2 do SWZ) oraz w Projekcie Umowy (załącznik nr 5 do SWZ).
