Przedmiotem zamówienia jest zakup wraz z dostawą na zlecenie Zamawiającego wyrobów medycznych pn.„DOSTAWA WYROBÓW MEDYCZNYCH CZ. V - PRODUKTY DO INTENSYWNEJ TERAPII, ANESTEZJOLOGII, BLOKU OPERACYJNEGO ORAZ SYSTEM OCZYSZCZANIA RAN”, szczegółowo opisanych w załączniku nr 2 do SWZ - Formularz asortymentowo-cenowy oraz w załączniku nr 6 do SWZ - Projektowane postanowienia umowne. Przedmiot zamówienia został podzielony na 13 części, zwanych dalej „Pakietami”. Zamawiający dopuszcza składanie ofert częściowych. W ramach zamówienia wyodrębniono następujące Pakiety: Pakiet nr 1 - czujniki do pulsoksymetru. Pakiet nr 2 - elektrody EKG. Pakiet nr 3 - przetworniki do pomiaru ciśnienia. Pakiet nr 4 - czujnik do pomiaru rzutu serca. Pakiet nr 5 - elektrody czynne i bierne do koagulacji. Pakiet nr 6 - elektrody bierne. Pakiet nr 7 - zestawy do pomiaru rzutu serca. Pakiet nr 8 - strzykawki bezpieczne. Pakiet nr 9 - system pomiaru ciśnienia w jamie brzusznej. Pakiet nr 10 - glukometr z paskami i płynem kalibrującym. Pakiet nr 11 - kaniula dotętnicza. Przedmiot zamówienia obejmuje dostawę wraz z transportem produktów zgodnie z opisem przedmiotu zamówienia oraz ilościami określonymi w załączniku nr 2 do SWZ - Formularzu asortymentowo-cenowym. Poszczególne Pakiety nie mogą być dzielone przez Wykonawców. Oferta musi obejmować pełny zakres danego Pakietu. Oferta nieobejmująca pełnego zakresu Pakietu zostanie odrzucona. Wykonawca może złożyć ofertę na wszystkie Pakiety lub na dowolną ich liczbę. Zamawiający nie ogranicza liczby Pakietów, na które Wykonawca może złożyć ofertę, ani liczby Pakietów, w zakresie których zamówienie może zostać udzielone temu samemu Wykonawcy.
