Pełen tytuł: Dostawa, instalacja i uruchomienie aparatów USG dla SPWSZ w Szczecinie zgodnie z dofinansowaniem wynikającym z umowy o powierzenie Grantu w ramach Projektu grantowego nr FERS.03.07-IP.07-0001/23 pod nazwą „Dostępność Plus dla AOS”, realizowanego w ramach programu Fundusze Europejskie dla Rozwoju Społecznego 2021-2027, współfinansowanego ze środków Europejskiego Funduszu Społecznego Plus na podstawie Decyzji o dofinansowaniu Projektu grantowego nr FERS.03.07-IP.07-0001/23-00 podjętej przez Ministra Zdrowia w dniu 08.12.2023 r. 1. Przedmiotem zamówienia jest dostawa, instalacja i uruchomienie aparatów USG dla SPWSZ w Szczecinie zgodnie z dofinansowaniem wynikającym z umowy o powierzenie Grantu w ramach Projektu grantowego nr FERS.03.07-IP.07-0001/23 pod nazwą „Dostępność Plus dla AOS”, realizowanego w ramach programu Fundusze Europejskie dla Rozwoju Społecznego 2021-2027, współfinansowanego ze środków Europejskiego Funduszu Społecznego Plus na podstawie Decyzji o dofinansowaniu Projektu grantowego nr FERS.03.07-IP.07-0001/23-00 podjętej przez Ministra Zdrowia w dniu 08.12.2023 r. 2. Wykonawca zobowiązany jest zrealizować zamówienie na zasadach i warunkach opisanych w projekcie umowy, stanowiącym załącznik nr 4 do SWZ oraz w Opisie przedmiotu zamówienia, stanowiącym załącznik nr 1 do SWZ i w załączniku nr 1A do SWZ. 3. Szczegółowy Opis Przedmiotu Zamówienia stanowi załącznik nr 1 do SWZ. 4. Miejscem realizacji przedmiotu zamówienia będzie Samodzielny Publiczny Wojewódzki Szpital Zespolony w Szczecinie z siedzibą przy ulicy Arkońskiej 4, 71-455 Szczecin. 5. Okres realizacji zamówienia: w terminie do 2 tygodni od daty zawarcia umowy, jednak nie później niż do dnia 30.11.2026 r. 1. Przedmiotem zamówienia jest dostawa, instalacja i uruchomienie aparatu USG kardiologicznego. 2. Szczegółowy Opis Przedmiotu Zamówienia dla Zadania nr 1 zawarty jest w załączniku nr 1 do SWZ dla Zadania nr 1 (Opis przedmiotu zamówienia - wymagane parametry). 3. Opis parametrów ocenianych dla Zadania nr 1 zawarty jest w załączniku nr 1A do SWZ dla Zadania nr 1.; 1. Przedmiotem zamówienia jest dostawa, instalacja i uruchomienie aparatu USG. 2.
