1. Przedmiotem zamówienia jest sukcesywna dostawa leków w ramach programu lekowego oraz środków farmaceutycznych. Zamówienie obejmuje dostawę wg asortymentu i w ilościach wskazanych na formularzu asortymentowo-cenowym, stanowiącym Załącznik nr 2 do SWZ, zawierającym szczegółowe wymagania dotyczące przedmiotu zamówienia. Przedstawione w nim ilości zawierają szacunkowe zestawienia na okres obowiązywania umowy. W rzeczywistości ilości te mogą być mniejsze, z zastrzeżeniem, że Zamawiający zrealizuje zamówienie w co najmniej 60% wartości zawartej umowy. Asortyment stanowiący przedmiot zamówienia musi spełniać wymagania zawarte w odpowiednich normach europejskich i być dopuszczony do obrotu zgodnie z obowiązującymi przepisami prawa.
