Przedmiot zamówienia stanowi: a) Transport pacjentów samochodem o standardzie odpowiadającym karetkom typu „S” z zapewnioną opieką medyczną (zespół z lekarzem) - Pakiet Nr 1 b) Transport pacjentów samochodem o standardzie odpowiadającym karetkom typu „T” z zapewnioną opieką medyczną (pielęgniarka lub ratownik medyczny) dla pacjentów: - SPSK-M w celu zachowania ciągłości leczenia (badania, konsultacje, kontynuacja leczenia itp.), - z dysfunkcją narządu ruchu uniemożliwiającą korzystanie ze środków transportu publicznego - Pakiet Nr 2 Transport pacjentów samochodem o standardzie odpowiadającym karetkom typu „S” z zapewnioną opieką medyczną (zespół z lekarzem); Transport pacjentów samochodem o standardzie odpowiadającym karetkom typu „T” z zapewnioną opieką medyczną (pielęgniarka lub ratownik medyczny) dla pacjentów: - SPSK-M w celu zachowania ciągłości leczenia (badania, konsultacje, kontynuacja leczenia itp.), - z dysfunkcją narządu ruchu uniemożliwiającą korzystanie ze środków transportu publicznego
