1. Przedmiotem zamówienia jest grupowe ubezpieczenie na życie pracowników Lubuskiego Urzędu Wojewódzkiego oraz członków ich rodzin. 2. Szczegółowy zakres zamówienia oraz wszystkie wymagania jakościowe świadczonych usług zostały określone w Szczegółowym opisie przedmiotu zamówienia, który stanowi załącznik nr 2 do SWZ. 3. Szczegółowy sposób realizacji przedmiotu zamówienia został określony w projektowanych postanowieniach umowy - załącznik nr 9 do SWZ. 4. PRAWO OPCJI: Zamawiający przewiduje, na podstawie art. 441 ustawy, możliwość skorzystania z prawa opcji w zakresie przedłużenia okresu obowiązywania ochrony ubezpieczeniowej w zakresie ubezpieczenia na życie do 2 (dwóch) lat, na zasadach określonych w projektowanych postanowieniach umowy - załącznik nr 9 do SWZ.
