Przedmiotem zamówienia jest usługa pobierania, opracowania, przygotowania i udostępnienia materiału biologicznego i danych klinicznych pochodzących od pacjentów z rakiem jelita grubego, rakiem piersi, niedrobnokomórkowym rakiem płuca i czerniakiem skóry. 2. Szczegółowy opis przedmiotu zamówienia znajduje się w Załączniku nr 3 do SWZ Opis przedmiotu zamówienia. 3. Szczegółowy zakres wykonania przedmiotu zamówienia znajduje się we wzorze umowy, stanowiącym Załącznik nr 2 SWZ. 4. Klasyfikacja wg kodu CPV: kod główny: 85141000-9 - Usługi świadczone przez personel medyczny kody pomocnicze: 85111800-8 - usługi patologiczne, 85140000-2 -różne usługi w zakresie zdrowia, 73110000-6 Usługi badawcze 5. Oferta musi być jednoznaczna i kompleksowa, tj. obejmować wszystkie usługi niezbędne do zrealizowania przedmiotu zamówienia. 6. Ze względu na konieczność zapewnienia odpowiedniej jakości materiału biologicznego do dalszych analiz oraz kluczowy wpływ czasu transportu na zachowanie jego parametrów jakościowych, ośrodek Wykonawcy musi znajdować się w odległości nie większej niż 50 km od siedziby Zamawiającego (ul. Na Grobli 15, 50-421 Wrocław), mierzonej najkrótszą trasą przejazdu samochodem przy wykorzystaniu aplikacji Google Maps. 1.
