1. Przedmiotem zamówienia jest świadczenie usług medycznych z zakresu dodatkowej opieki medycznej dla pracowników spółki Aplikacje Krytyczne Sp. z o.o. i ich rodzin w formie następujących pakietów: 1) Pakiet indywidualny I - pracownik Zamawiającego; 2) Pakiet partnerski II - pracownik Zamawiającego oraz osoba bliska pracownika; 3) Pakiet rodzinny III - pracownik Zamawiającego oraz osoby bliskie pracownika; Zamawiający wymaga, aby Wykonawca posiadał w województwie mazowieckim minimalną liczbę 500 placówek (własnych lub współpracujących) oraz co najmniej 1500 placówek w pozostałych województwach Polski, świadczących usługi medyczne. Do liczby placówek nie są brane pod uwagę placówki oferujące usługi tylko w zakresie medycyny pracy. Zamawiający zastrzega sobie prawo do weryfikacji zakresu działalności poszczególnych placówek. Wykonawca jest zobowiązany wskazać liczbę placówek w wykazie stanowiącym załącznik do Formularza ofertowego. Niespełnienie ww. wymogu będzie skutkowało odrzuceniem oferty. Zamawiający przewiduje możliwość skorzystania z prawa opcji. Zamówienie podstawowe: Świadczenie usług medycznych z zakresu dodatkowej opieki medycznej dla pracowników spółki Aplikacje Krytyczne Sp. z o.o. (Zamawiającego) i ich rodzin przez okres 36 miesięcy od dnia podpisania umowy. Zamówienie w ramach prawa opcji: Świadczenie usług medycznych z zakresu dodatkowej opieki medycznej dla pracowników spółki Aplikacje Krytyczne Sp. z o.o. (Zamawiającego) i ich rodzin przez okres maksymalnie 12 miesięcy od dnia zakończenia umowy. 2. Szczegółowy opis przedmiotu zamówienia zawarty jest w załączniku nr 1 do SWZ - Opis Przedmiotu Zamówienia (OPZ).
