Description

1. Przedmiotem zamówienia są usługi cateringowe świadczone dla jednostek organizacyjnych Uniwersytetu Medycznego w Lublinie. 2. Szczegółowy opis przedmiotu zamówienia zawarty jest w Załączniku nr 1 do SWZ „Opis przedmiotu zamówienia” (zwanym również OPZ). 5.1. Przedmiot zamówienia obejmuje następujący zakres i ilość gwarantowaną (dalej: zakres gwarantowany): 5.1.1. napoje zimne - ilość gwarantowana 500 sztuk 5.1.2. napoje gorące - ilość gwarantowana 500 sztuk 5.1.3. zimne przekąski (bez obsługi) - ilość gwarantowana 250 sztuk 5.1.4. desery (bez obsługi) - ilość gwarantowana 250 sztuk 5.1.5. przerwa kawowa I - ilość gwarantowana 2500 sztuk 5.1.6. przerwa kawowa II - ilość gwarantowana 200 sztuk 5.1.7. lunch - ilość gwarantowana 300 sztuk 5.1.8. catering okolicznościowy - ilość gwarantowana 100 sztuk 5.1.9. catering okolicznościowy wigilijny - ilość gwarantowana 200 sztuk 5.1.10. catering okolicznościowy wielkanocny - ilość gwarantowana 200 sztuk 5.1.11. lunch na wynos - ilość gwarantowana 500 sztuk 5.2. Zamawiający przewiduje skorzystanie z prawa opcji, o którym mowa w art. 441 ust. 1 ustawy Pzp. Prawem opcji objęta zostanie usługa obejmująca zakres poszczególnych usług cateringowych (zestawów) wskazanych w pkt 5.1 powyżej, o łącznej wartości nie przekraczającej 140% wartości brutto wskazanej w ofercie Wykonawcy w zakresie zamówienia gwarantowanego. 3. Zamawiający nie wymaga od Wykonawcy złożenia wraz z ofertą przedmiotowych środków dowodowych. 4. Do oferty Wykonawca zobowiązany jest dołączyć aktualne na dzień składania ofert oświadczenie o niepodleganiu wykluczeniu oraz spełnianiu warunków udziału w postępowaniu w formie Jednolitego Europejskiego Dokumentu Zamówienia (JEDZ) wg załącznika nr 3 do SWZ 5. RODO (obowiązek informacyjny i obowiązek stosowania): Informacje dotyczące ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych określa rozdział XXIII SWZ 6. Wykonawca zobowiązany jest zrealizować zamówienie na zasadach i warunkach opisanych w projektowanych postanowieniach umowy stanowiących:
