Description

Przedmiotem zamówienia jest dostawa leku Dinutuximabum beta w ramach RDTL. Szczegółowy opis oraz sposób realizacji zamówienia zawiera tabela asortymentowo-cenowa stanowiący zał. nr 2 do SWZ, wzór umowy stanowiący zał. nr 4 do SWZ. Zakup finansowany w ramach procedury Ratunkowego Dostępu do Technologii Lekowych (RDTL). Zamawiający nie dopuszcza możliwości składania ofert częściowych. Przedmiotem zamówienia jest tylko jedna substancja lecznicza i w związku z tym nie ma możliwości podziału zamówienia na części. Zamawiający oświadcza, iż będąc administratorem danych oraz mając na względzie przepisy ustawy z dn.10.05.2018 r. o ochronie danych osobowych (dalej RODO) wdrożył środki bezpieczeństwa adekwatne do zidentyfikowanych przez siebie ryzyk oraz dające rękojmie należytego zabezpieczenia danych osobowych przekazywanych w ramach postępowania. Szczegółowa klauzula informacyjna z art. 13 RODO zawarta jest w SWZ.
