Description

1. Przedmiotem zamówienia jest robota budowlana polegająca na przebudowie dwóch budynków CZP w Słupsku celem rozszerzenia działalności Oddziału Dziennego Psychiatrycznego dla dorosłych i uruchomienie Oddziału Dziennego Rehabilitacyjnego dla dzieci. Decyzja budowlana nr 90/2024 z dnia 24.06.2024 r. wydana przez Prezydenta Miasta Słupska sygnatura B.6740.51.2024.JG została wydana na przebudowę dwóch budynków poszpitalnych (o numerach ewid. 374 i 375) na potrzeby Centrum Zdrowia Psychicznego w Słupsku wraz z zagospodarowaniem terenu i infrastrukturą towarzyszącą (przebudowa układu komunikacji wewnętrznej wraz z budową parkingów dla samochodów osobowych, zewnętrzna instalacja wodociągowa, kanalizacji sanitarnej, kanalizacji deszczowej z podziemnym zbiornikiem retencyjnym, ciepłownicza, elektroenergetyczna z oświetleniem terenu i monitoringiem, instalacja fotowoltaiczna, wiata śmietnikowa, boisko do gry w siatkówkę, siłownia zewnętrzna, rewitalizacja historycznego założenia parkowego), na działkach nr: 1062/3 i 1062/4 w obrębie ewid. nr 13 przy ul. prof. Lotha oraz 1056 w obrębie ewid. nr 13 przy ul. Obrońców Wybrzeża w Słupsku. 2. Szczegółowy opis przedmiotu zamówienia opisany jest w dokumentacji projektowej, na którą składają się projekt budowlany, specyfikacje techniczne wykonania i odbioru robót budowlanych (STWiORB) - dalej dokumentacja projektowa. Przedmiary robót mają charakter pomocniczy. 3. Warunki realizacji zamówienia określa również wzór umowy (Część II SWZ).
