1.Przedmiotem zamówienia są sukcesywne dostawy leków, w tym leków z programów lekowych wraz z użyczeniem sprzętu tj. 2 pomp do prowadzenia znieczulenia metodą docelowego stężenia osoczowego TCL (dotyczy zadania 14 poz.2) na potrzeby Wojewódzkiego Szpitala Specjalistycznego im. Marii Skłodowskiej-Curie w Zgierzu. Oferta Wykonawcy musi obejmować pełen zakres zamówienia tj. 100% asortymentu określonego w formularzu asortymentowo-cenowym dla danego zadania. Złożenie oferty nieobejmującej wszystkich pozycji w danym zadaniu, będzie skutkowało jej odrzuceniem jako niezgodnej z warunkami zamówienia. 2.Zamówienie obejmuje 29 zadań (części). Zadania 23 - 29 - leki z programów lekowych, gdzie Zadanie 23 - program lekowy C.89, zadanie 24 - program lekowy B.29, zadanie 25 - program lekowy B.67, zadanie 26 - program lekowy B.70, zadanie 27 - program lekowy B.99, zadanie 28 - program lekowy C.45a, zadanie 29 - program lekowy B.113 3.Szacunkowy wykaz dostaw asortymentu na czas obowiązywania umowy zawiera załącznik nr 2 do SWZ.
