Dostawa leku Kalydeco w ilości 2500 opakowań, rozliczanego w ramach programu lekowego. Przedmiot zamówienia musi spełniać wymagania ustawy z dnia 6 września 2001 r. - Prawo farmaceutyczne. Zakup jest realizowany w ramach Programu lekowego B.112 refundowanego przez NFZ pn. „Leczenie Mukowiscydozy”. Ivacaftorum 150 mg x 28 tabletek (Kalydeco) lub w zależności od potrzeb Zamawiającego: a) Ivacaftorum 75 mg x 28 tabletek (Kalydeco), b) Ivacaftorum 75 mg x 28 saszetek (Kalydeco), c) Ivacaftorum 59,5 mg x 28 saszetek (Kalydeco), w ilości 2500 opakowań.
