Dostawa leku Kaftrio w ilości 3650 opakowań, rozliczanego w ramach programu lekowego. Przedmiot zamówienia musi spełniać wymagania ustawy z dnia 6 września 2001 r. - Prawo farmaceutyczne. Zakup jest realizowany w ramach Programu lekowego B.112 refundowanego przez NFZ pn. „Leczenie Mukowiscydozy”. Ivacaftorum 75mg + Tezacaftorum 50mg + Elexacaftorum 100mg x 56 tabletek (Kaftrio) lub w zależności od bieżących potrzeb Zamawiającego: a) Ivacaftorum 37,5mg + Tezacaftorum 25mg + Elexacaftorum 50mg x 56 tabletek (Kaftrio), b) Ivacaftorum 75mg + Tezacaftorum 50mg + Elexacaftorum 100mg x 28 saszetek (Kaftrio), c) Ivacaftorum 60mg + Tezacaftorum 40mg + Elexacaftorum 80mg x 28 saszetek (Kaftrio). w ilości 3650 opakowań.
