Description

Przedmiotem zamówienia jest dostawa leków, preparatów diagnostycznych, substancji recepturowych i art. pomocniczych stosowanych w recepturze materiałów opatrunkowych dla Samodzielnego Publicznego Zakładu Opieki Zdrowotnej w Sokółce. Szczegółowy opis przedmiotu zamówienia oraz ilości zawiera Załącznik nr 2 do SWZ - Formularz asortymentowo - cenowy. Szczegółowe warunki realizacji przedmiotu zamówienia określa projekt umowy stanowiący Załącznik 3 do SWZ. Na podstawie art. 441 Pzp zamawiający przewiduje opcję o maksymalnej wartości 40%, tj. dopuszcza się możliwość zwiększenia do 40% dostaw przedmiotu zamówienia w stosunku do podstawowego zakresu, w przypadku zwiększonego zapotrzebowania np. w wyniku wzrostu liczby pacjentów, zwiększonej częstotliwości stosowania przedmiotu zamówienia, w przypadku opóźnień lub przedłużających się procedur związanych z przygotowaniem, prowadzeniem lub zakończeniem kolejnego postępowania o udzielenie zamówienia w zakresie przedmiotu tożsamego z przedmiotem niniejszego postępowania lub w innym przypadku, którego zamawiający nie mógł przewidzieć. Zamawiający zastrzega również, że Wykonawcy nie przysługuje żadne roszczenie w stosunku do Zamawiającego w przypadku, gdy Zamawiający z prawa opcji nie skorzysta. Zamawiający zastrzega również możliwość niezrealizowania do 20% przedmiotu umowy, w takim przypadku Wykonawca może żądać wyłącznie wynagrodzenia należnego z tytułu wykonania odpowiedniej części umowy, bez naliczania jakichkolwiek kar lub dochodzenia roszczeń z tego tytułu. Minimalna wartość umowy jaka zostanie wykonana wynosi 80% w stosunku do podstawowego zakresu zamówienia.
