Description

Przedmiotem zamówienia są sukcesywne dostawy materiałów opatrunkowych, rękawic medycznych oraz innych wyrobów medycznych dla Zespołu Opieki Zdrowotnej w Bolesławcu. Szczegółowy opis przedmiotu zamówienia zawiera Formularz cenowy - Załącznik nr 2.1-2.16 do SWZ. Każdy Wykonawca może złożyć tylko jedną ofertę na całość zamówienia lub dane zadanie/ zadania w formie pisemnej pod rygorem nieważności. W przypadku złożenia przez Wykonawcę więcej niż jednej oferty na całość zamówienia lub na dane zadanie/ zadania wszystkie oferty tego Wykonawcy zostaną odrzucone przez Zamawiającego. Zamówienie składa się z 16 zadań. Zamawiający dopuszcza składanie ofert częściowych. Za ofertę częściową Zamawiający uzna ofertę złożoną na wszystkie pozycje w danym zadaniu. Zamawiający nie dopuszcza składania ofert na pojedyncze pozycje. Zamawiający zastrzega, że oferowany przedmiot zamówienia musi należeć do grupy wyrobów medycznych w rozumieniu ustawy z dnia 7 kwietnia 2022 r. o wyrobach medycznych (Dz. U. z 2022 r., poz. 974) i Rozporządzenia Parlamentu Europejskiego i Rady UE 2017/745 z dnia 5 kwietnia 2017r. w sprawie wyrobów medycznych, zmiany Dyrektywy 2001/83/WE, Rozporządzenia (WE) nr 178/2002 i Rozporządzenia (WE) nr 1223/2009 oraz uchylenia dyrektyw Rady 90/385/EWG i 93/42/EWG (z wyłączeniem poz. 2 w Zadaniu nr 3). Oferowany przedmiot zamówienia musi posiadać wszelkie niezbędne dopuszczenia do użytkowania w placówkach służby zdrowia zgodnie z polskim prawem oraz prawem Unii Europejskiej.
