Description

1. Przedmiotem zamówienia jest: zakup i dostawy sukcesywne sprzętu medycznego jednorazowego użytku. 2. Szczegółowy opis przedmiotu zamówienia pod względem wymaganych parametrów został wyszczególniony w załącznikach nr 2.1 - 2.7 do SWZ - Opis Przedmiotu Zamówienia (część zamówienia/pakiet/zadanie nr 1 - 7) - określanym również „załącznikiem nr 2 do SWZ”. 3. Miejsce realizacji zamówienia: ul. Grenadierów 51/59, 04-073 Warszawa. 4. Dostawy sukcesywne przedmiotu zamówienia mają być realizowane do Apteki Szpitalnej mieszczącej się w Pawilonie III Szpitala na parterze i do magazynu znajdującego się w podziemiach Szpitala, przy czym każdorazowo Zamawiający wskaże, jeśli miejscem dostawy będzie magazyn. 5. Odbiór przedmiotu zamówienia odbywać będzie się w pomieszczeniach dostępnych dla pracowników Wykonawcy (dostawców, kurierów). 6. Szczegółowe warunki realizacji usługi określa projekt umowy stanowiący załącznik nr 4 do SWZ. 7. Zamawiający dopuszcza możliwości składania ofert częściowych (7 - zadań / części zamówienia), zgodnie z podziałem określonym w załączniku nr 2 do SWZ stanowiącym formularzem Opis przedmiotu zamówienia: 1) Zadanie nr 1 - Rampa; 2) Zadanie nr 2 - Przewód oddechowy; 3) Zadanie nr 3 - Zestaw do zbiórki i worki; 4) Zadanie nr 4 - Jednorazowy układ pacjenta; 5) Zadanie nr 5 - Sprzęt endoskopowy; 6) Zadanie nr 6 - Igły EUS; 7) Zadanie nr 7 - Zawór ssący.
