Description

Przedmiotem zamówienia jest dostawa materiałów kardiologicznych do zabiegów sercowych wraz z dzierżawą urządzenia. Szczegółowy opis przedmiotu zamówienia zawiera arkusz cenowy (zał. nr 1a do SWZ). Opis ten należy odczytywać wraz z ewentualnymi zmianami treści SWZ, będącymi np. wynikiem udzielonych odpowiedzi na zapytania Wykonawców. Podane ilości są szacunkowym zapotrzebowaniem i służą do obliczenia ceny oferty (tj. ustalenia maksymalnego wynagrodzenia wykonawcy). Zamawiający zastrzega sobie prawo rezygnacji z zakupu części asortymentu wynikającej z braku lub zmniejszenia zapotrzebowania. 1. Sterylna jednorazowa osłona - 4500 sztuk 2. Przewody wodne - 1 350 sztuk 3. Rozdzielacze sygnału 'typ Y' - 30 sztuk; 1. Kardiowerter defibrylator resynchronizujący z kompletem elektrod i cewników - 450 sztuk; Rozrusznik podstawowy resynchronizujący ze złączem lewokomorowym IS-1 - 270 sztuk; 1. Rozrusznik RESYNCHRONIOZUJĄCY do stymulacji lewokomorowej dwupunktowej, przystosowany do badania rezonansem, z kompletem elektrod i cewników - 150 sztuk; 1. Kardiowerter defibrylator jednojamowy (VVI) wraz z elektrodą: 1) Typ A - 180 sztuk 2) Typ B - 30 sztuk; 1. Kardiowerter defibrylator DDD z kompletem elektrod: 1) Typ A - 210 sztuk 2) Typ B - 180 sztuk; 1. Rozrusznik dwujamowy (DDDR) z kompletem elektrod - 1 650 sztuk 2. Złącze obrotowe - 100 sztuk; 1. Rozrusznik jednojamowy (VVIR) z kompletem elektrod - 420 sztuk; 1. Rozrusznik dwujamowy (DDDR) o zwiększonej pojemności baterii z kompletem elektrod - 150 sztuk 2. Implantowalny rozrusznik do bezelektrodowej stymulacji serca VVI - 6 sztuk; 1. Wkłucia do żyły podobojczykowej - koszulki rozrywane dedykowane do implantacji elektrod rozrusznikowych - 4 500 sztuk; 1. Elektroda do defibrylatora - 900 par; 1. Prowadnik diagnostyczny - 1 650 sztuk 2. Wkłucie do naczyniowe tętnicze i żylne - 300 sztuk
