Description

1. Przedmiotem zamówienia są: Dostawy materiałów medycznych ortopedycznych. Zamówienie podzielone jest na 11 pakietów (części). Umowy w zakresie rozstrzygniętych pakietów zostaną zawarte po zakończeniu niniejszego postępowania na okres 12 miesięcy. 2. Szczegółowy opis przedmiotu zamówienia zawierają załączniki: 1) Formularze cenowe (odrębnie dla każdego z pakietów) - załącznik nr 1.1 do SWZ, 2) Projektowane postanowienia umowy, które zostaną wprowadzone do treści umowy /postanowienia umowy - załącznik nr 2 do SWZ. 3. Zamawiający informuje, iż zgodnie z art. 139 PZP najpierw dokona badania i oceny ofert, a następnie dokona kwalifikacji podmiotowej Wykonawcy, którego oferta została najwyżej oceniona, w zakresie braku podstaw do wykluczenia oraz spełniania warunków udziału w postępowaniu. 4. Realizując obowiązek wynikający z art. 24 ust. 6 ustawy z dnia 14 czerwca 2024r. o ochronie sygnalistów zamawiający informuje, że w Samodzielnym Publicznym Zakładzie Opieki Zdrowotnej Wojewódzkim Szpitalu Specjalistycznym Nr 3 w Rybniku przyjęta została procedura wewnętrznych zgłoszeń informacji o naruszeniach prawa i podejmowania działań następczych.
