Description

Het expertisecentrum ondersteunt de uitvoering van de regionale visie Wonen doe je thuis en heeft als doel om uithuisplaatsingen zoveel mogelijk te voorkomen en, wanneer verblijf buiten het gezin noodzakelijk is, bij te dragen aan de totstandkoming van duurzame en perspectief biedende woonoplossingen voor jeugdigen. De opdracht omvat drie samenhangende functies: · het begeleiden van de zoektocht naar een 'tweede thuis dichtbij' binnen het eigen netwerk of de leefwereld van de jeugdige; · het bieden van overzicht en advies aan gezinnen en verwijzers over passend woonperspectief binnen het regionale woonlandschap; · het versterken van vakmanschap en regionale samenwerking door scholing, consultatie, kennisdeling en praktijkondersteuning op het gebied van circulair samenwerken. Het expertisecentrum richt zich op jeugdigen en gezinnen waarbij sprake is van een (dreigende) uithuisplaatsing en waar behoefte is aan een duurzame woon- of opvoedplek. De geraamde contractwaarde bedraagt € 92.000 voor de implementatiefase in 2026 en € 370.000 per jaar vanaf 2027.
