Procedura Aperta per la conclusione di un accordo quadro con un operatore economico per lotto per la progettazione condivisa e partecipata e la cogestione di Progetti Terapeutico Riabilitativi Individualizzati (PTRI) con Budget di Salute (BdS), servizio di supporto accompagnamento individuale e di mediazione sociale per l’abitare autonomo, servizio di prescrizione sociale, progetti di convivenza sull’abitare di transizione e di progetti di attività di comunità (PACo) per il Dipartimento Salute Mentale e Dipendenze Patologiche dell’Azienda USL di Bologna, suddivisa in quattro lotti Progetti individualizzati abilitativi educativi e/o socio-riabilitativi con Budget di Salute (CSM, NPIA); Progetti individualizzati abilitativi educativi e/o socio-riabilitativi con Budget di Salute (SerDP); Progetti di convivenza sull’abitare di transizione (CSM); Attività di comunità (DSM DP)
