Digitale Nachsorgeplattform zur Nachversorgung von Patienten nach dem Krankenhausaufenthalt Der Landkreis Gießen ist eines der 73 "Modellprojekte Smart Cities" (MPSC), die seit 2019 von der Bundesregierung gefördert werden und sektorenübergreifende, nachhaltige Digitalstrategien erarbeiten und erproben. Gegenstand des vorliegenden Verfahrens ist die Beschaffung, Einfüh-rung und der Betrieb einer webbasierten, sektorenübergreifenden digitalen Nachsorgeplattform einschließlich mobiler App zur strukturierten Betreuung geriatrischer und pflegebedürftiger Patientinnen und Patienten nach Krankenhausentlassung, beginnend mit einer Pilotierung für zunächst etwa 50 Patientinnen und Patienten an der Asklepios Klinik in Lich im Landkreis Gie-ßen. Ziel ist die Reduktion vermeidbarer Wiederaufnahmen sowie die priorisierte, indikatoren-gestützte Nachsorge durch kontinuierliches Monitoring, automatisierte Patientenkategorisie-rung im Ampelsystem (grün/gelb/rot) und ein virtuelles Care Team, das telemedizinische Über-wachung und Intervention ermöglicht. Das Vorhaben "Licher Modell - Digitale Nachsorge" wird im Programm "Starke Heimat Hessen" gefördert und adressiert u. a. demografische und versor-gungsstrukturelle Herausforderungen im ländlichen Raum, indem Klinik, Haus- und Fachärzte, Pflege, Angehörige und Ehrenamt eng vernetzt werden. Die Projektsprache ist Deutsch. Alle Beratungsleistungen müssen in deutscher Sprache angebo-ten werden. Ebenso müssen die Projektdokumente und -dokumentationen in deutscher Sprache verfasst werden.
