Description

Gegenstand der Beschaffung ist die Entwicklung, Koordination und Durchführung sowie die Evaluation eines Programms zur Gesundheitsförderung und Prävention für Grund- und Förderschulen (Fachlos 2). Die vom Auftragnehmer zu erbringenden Leistungen lassen sich grob in mehrere Schritte (Phasen) untergliedern, die erst nach Zuschlagserteilung mit dem Auftragnehmer und der DAK-Gesundheit feinabgestimmt werden: - Schritt 1: Aufbau der Arbeitsstruktur / Erstellen eines Projektplans - Schritt 2: Schritt 2: Konzeptions- und Entwicklungsphase - Schritt 3: Implementierungsphase - Schritt 4: Umsetzungs- und Evaluationsphase Gegenstand des Auftrags ist die Erstellung verschiedener Konzepte. Die Konzepte haben die Ausgangslage, die Zielsetzung und Zielgruppen des Vorhabens einzuordnen, den Lösungsansatz, die erforderlichen Ressourcen, eine Zeitplanung, Risiken und Erfolgsfaktoren sowie eine klare und nachvollziehbare Planung und Vorgehensweise (Maßnahmen und Methodik) des Auftragnehmers zu beschreiben. Die Konzepte müssen schlüssig und für andere nachvollziehbar, d.h. verständlich und prägnant geschrieben sein. Folgende Konzepte sind nach der Zuschlagserteilung vom Auftragnehmer zu erstellen: 1. Programmkonzept inkl. Schulungskonzept: bis zum 30.06.2027 gemäß Abschnitt 2.2.1 der Leistungsbeschreibung 2. Websitekonzept: innerhalb von 10 Tagen im Anschluss an den Workshop gemäß Abschnitt 2.2.2 der Leistungsbeschreibung 3. Evaluationskonzept: Grobkonzept innerhalb von 10 Tagen im Anschluss an den Workshop gemäß Abschnitt 2.2.3 der Leistungsbeschreibung, Ausarbeitung Evaluationskonzept bis zum 30.06.2027 Die Einzelheiten können der Leistungsbeschreibung zum Fachlos 1 (Anlage Nr. B1) entnommen werden.
