Description

Die Stadt Hamm schreibt im Namen und auf Rechnung der Kommunales Jobcenter Hamm AöR (Auftraggeber) eine Maßnahme (gemäß § 16 k SGB II) zur ganzheitlichen Betreuung für SGB II Leistungsbeziehende in einem offenen Verfahren aus. Leistungsgegenstand ist die Durchführung einer Maßnahme auf Basis der Rechtsgrundlage § 16 k zweites Sozialgesetzbuch (SGB II). Hauptzielsetzung ist die Rückführung der Teilnehmenden in das Regelsystem. Die Maßnahme wird als erfolgreich bewertet, wenn mindestens 80 Prozent der geförderten Teilnehmenden zum individuellen Ende der ganzheitlichen Betreuung - wieder an Beratungsgesprächen des jeweiligen Mitarbeitenden des Auftraggebers teilnehmen und im Rahmen eines Kooperationsplanes Handlungsschritte für die weitere Entwicklung vereinbaren. Teilnehmende sind erwerbsfähige Leistungsberechtigte aus dem Rechtskreis des SGB II. Die Personen sind für die herkömmliche Beratung nicht zugänglich und zudem von komplexen Problemlagen betroffen, die in vielen Fällen nicht vollständig bekannt sind. Es ist möglich, dass einzelne Personen oder Mitglieder der Bedarfsgemeinschaft zugewiesen werden sowie alle Mitglieder der Bedarfsgemeinschaft Teilnehmende sind. Einige Teilnehmende können über eingeschränkte Kenntnisse der deutschen Sprache verfügen.
