Description

Účelem veřejné zakázky je uzavření rámcové dohody s jedním na dodavatelem na provádění stavebních prací, oprav povrchů komunikací v katastrálním území Frenštát pod Radhoštěm dle požadavků objednatele stanovených dále ve smlouvě a vyplývajících ze zadávací dokumentace na veřejnou zakázku a/nebo platných právních předpisů a ČSN norem. Předmětem smlouvy je provádění zejména těchto činností: - zpracování dokumentace pro provádění stavby (DPS) nebo technického řešení dle charakteru opravy (technické řešení by mělo obsahovat: mapový podklad se zákresem rozsahu prováděných prací; textový popis a položkový rozpočet včetně jednotkových cen); - odstranění asfaltobetonového krytu komunikace včetně odvozu na skládku do 5km nebo likvidace s následným doložením vážních listů a prohlášení o likvidaci nebo recyklaci; - „doložení dokladů o zákonné likvidaci, uložení nebo recyklaci všech druhů stavební suti a odpadů vzniklých při realizaci díla, a to včetně předložení příslušných vážních lístků; zajištění odběru vzorků a laboratorního posouzení vyfrézované asfaltové směsi (R-materiálu) akreditovanou laboratoří k určení kvalitativního a ekologického zatřídění (zejména obsahu PAU); - pokládka nového asfaltobetonového povrchu včetně zalití pracovních spár; - doplnění a obnova jednotlivých konstrukčních vrstev; - úprava a doplnění přídlažby, obrub, kanalizačních šachet a vpustí a jejich napojení, výšková úprava vodovodní, plynovodních ventilů a plochy přilehlých vjezdů a chodníkových těles, zřízení krajnic z asfaltobetonového recyklátu, usazení příčného odvodnění a žlabových tvárnic; - úprava stavbou dotčených nezpevněných ploch, terénní úpravy, ohumusování a osetí travním semenem; - zajištění ochrany kořenových systémů a kmenů stromů v ochranném pásmu komunikace a ochrana stávajících sítí proti poškození včetně projednání s dotčenými orgány a vlastníky; - obnova nebo nová realizace trvalého vodorovného a svislého dopravního značení; - obnova a zřízení zpomalovacích prahů; - zajištění dočasného dopravního značení k dopravnímu omezení; - u jednotlivých správců inženýrských sítí poptání vyjádření k existenci sítí a následné objednání vytýčení sítí v terénu; - zajištění stanoviska ke zvláštnímu užívání a umístění dopravního značení u příslušného oddělení dopravního inspektorátu PČR a Městského úřadu Frenštát pod Radhoštěm, odboru stavebního úřadu a územního plánování; - zajištění projednání a získání souhlasného stanoviska k dočasnému přesunu autobusových zastávek u dotčených provozovatelů veřejné linkové dopravy a příslušných dotčených orgánů, včetně fyzické realizace tohoto přesunu a osazení náhradních označníků; - zajištění upozornění občanů přímo dotčených stavebními pracemi; - zhotovení geodetického zaměření skutečného provedení stavby oprávněným geodetem; - umožnit zaměstnancům nebo zmocněncům poskytovatele dotace, kontrolního orgánu či objednatele vstup do objektu zhotovitele a provést kontrolu dokladů souvisejících se stavebními pracemi; - vedení stavebního deníku ode dne zahájení stavebních prací, který bude denně přístupný v místě plnění; do stavebního deníku je zhotovitel povinen čitelně zapisovat všechny skutečnosti rozhodné pro plnění dílčí smlouvy; - pořízení fotodokumentace plnění před zahájením stavebních prací, v jejich průběhu a po dokončení plnění; - doložení prohlášení o shodě, atestů, certifikátů a dalších relevantních dokladů prokazujících požadované vlastnosti a jakost u všech materiálů a výrobků zabudovaných do díla.
