Het Algemeen Ziekenhuis AZ Groeninge Kortrijk wenst over te gaan tot een overeenkomst voor aankoop en levering van geprebarcodeerd afnamemateriaal voor de collectie van bloed, urine, faesces, vochten en andere lichaamsmaterialen. De overeenkomst wordt afgesloten voor een termijn van 4 jaar, met de mogelijkheid om daarna 4 maal voor een periode van 1 jaar stilzwijgend te verlengen. De opdracht is opgedeeld in volgende percelen: - Perceel Nr. 1: Bloedbuizen, naalden en naaldhouders; - Perceel Nr. 2: Urinecontainers en -buizen; - Perceel Nr. 3: Pediatrisch bloedafnamemateriaal; - Perceel Nr. 4: Bloedgasspuiten; - Perceel Nr. 5: Diversen (o.a. potjes voor faescescollectie, sputum, …); - Perceel Nr. 6: Het aanleveren van labels met unieke barcodes en het aanbrengen van de labels op de artikels van de percelen 3, 4 en 5, alsook op de wissers die in het klinisch laboratorium gebruikt worden. Voor perceel 1 & 2 dienen de buizen bij productie van unieke barcodes voorzien te zijn. Voor perceel 3, 4 en 5 kunnen de materialen eventueel ook achteraf gebarcodeerd worden. Dit kan door de leverancier van het afnamemateriaal zelf gebeuren, of door een derde partij (zie perceel 6). Gezien de heterogeniteit van perceel 5 zal, i.t.t. de andere percelen, dit perceel aan meerdere leveranciers worden toegewezen. Het dient mogelijk te zijn de bestellingen te laten plaatsen door de 2 verschillende activiteitencentra binnen de organisatie, AZ Groeninge Kortrijk en de Sint-Jozefskliniek Izegem, met afzonderlijke leveringen. De opdrachten binnen deze overeenkomst zullen uitgevoerd worden op afroep, aan de hand van afzonderlijke bestelbonnen.
